The Family Education Rights and Privacy Act (FERPA) affords parent/guardians of children receiving services under EI or CPSE certain rights with respect to their child's record. Our FERPA policy is available here for you to download, and it will be maintained here on our web site for your review. Additionally, forms referred to in our FERPA policy are available here for download including: Medical Record Access Request Form , Medical Record Copy Request Form , Amendment Request Form, Access Log Request Form and Optional Information Release Form .
